We are happy to present to you our new 2025 intake application.
- benefit from circle of care sessions leading up to intake day. These circle of care sessions will be mandatory for acceptance and will be organized during the phone interview process. You will be granted a conditional acceptance letter that will be sent to referrals.
- Mawiomi has also partnered with the CHUM Addictions Medical team and every applicant will be reviewed for potential medical needs prior to entering the in-house program. This could mean that a referral(client) will have a medical assessment via zoom/phone.
- Require a printout of all medications from the pharmacy (best possible medication history).
- Medications to be in blister packs (dispill) as much as possible.
- Included in the new 2025 application process, is a diagram that shows the intake process in 5 stages.
